چکیده

Current treatment for midgut carcinoid includes radical surgery of the primary tumour, resection of lymph node metastases and resection of liver metastases. In the presence of disseminated disease however, cure is unusual. Palliative liver resection and hepatic artery embolisation are also recognised to decrease hormone secretion and reduce tumour volume. Somatostatin analogues reduce the synthesis and secretion of neuropeptides so reducing hormonal symptoms in up to 80% of patients. Alpha interferon has been previously shown to reduce urinary 5HIAA levels in carcinoid patients. In a previous Norwegian study a survival advantage was shown for patients treated with alpha interferon in combination with hepatic arterial embolisation. This present study was therefore carried out to identify the effect of alpha interferon added to a somatostatin analogue on survival and tumour progression after initial tumour reduction by surgery and embolisation for disseminated mid gut carcinoids. Sixty eight patients were treated with maximum tumour reduction and underwent hepatic artery embolisation followed by randomisation to either treatment with octreotide or a combination of octreotide and alpha interferon. All patients had liver metastases. The overall five year survival rate was 66%. Although there was no significant difference in survival between patients treated with octreotide alone (five year survival rate 37%) and those given octreotide in combination with alpha interferon (five year survival rate 57%), patients treated with alpha interferon had a significantly reduced risk of tumour progression during follow up (p = 0.008). This was a relatively small but very important study which demonstrates that alpha interferon may have a role in inhibiting tumour growth in patients with disseminated midgut carcinoid who have undergone debulking surgery and hepatic embolisation. Hopefully further larger studies with quality of life assessments included will be forthcoming.

6-Thioguanine treatment in inflammatory bowel disease: a critical appraisal by a European 6-TG working party.

Recently, the suggestion to use 6-thioguanine (6-TG) as an alternative thiopurine in patients with inflammatory bowel disease (IBD) has been discarded due to reports about possible (hepato) toxicity. During meetings arranged in Vienna and Prague in 2004, European experts applying 6-TG further on in IBD patients presented data on safety and efficacy of 6-TG. After thorough evaluation of its risk...

Optimizing treatment with thioguanine derivatives in inflammatory bowel disease.

Thioguanine derivatives, azathioprine and 6-mercaptopurine, represent major drugs in the treatment of chronic active inflammatory bowel disease. They are effective in two-thirds of the patients and safe over the long term in patients who can tolerate them (80-90%). Recent progress in understanding the metabolism of these drugs and its implication in clinical practice have brought up new tools a...

Measurement of thiopurine methyltransferase activity and azathioprine metabolites in patients with inflammatory bowel disease.

BACKGROUND Measurement of 6-thioguanine nucleotide concentrations may be useful for optimising treatment with azathioprine and 6-mercaptopurine. METHODS We conducted a study of 170 patients with inflammatory bowel disease treated with azathioprine or 6-mercaptopurine to determine the relationship between 6-thioguanine nucleotide concentrations and both disease activity, as measured by the inf...

Prevalence of Allergies and Food Intolerance: a Comparison Between the Persian Community and Canadians of European Descent

Background: The prevalence of reported allergic diseases is higher in North-America than in other countries around the world. Further, certain types of allergies are more common in some geographic regions or amongst different ethnic populations. This phenomenon has not been well studied amongst the Persian population in the Canadian context; therefore, the current study aimed to compare the pre...
